The English-Japanese Well-Baby/Child Visit Questionnaires ”日英併記乳幼児健診アンケート”
Regular checkups （乳幼児健康検査） are held at Health Centers in Sapporo. They help to monitor children’s health and development. When it is time for your child’s checkup, you will receive a notice from the City of Sapporo that includes the scheduled date and time. Fill out the English-Japanese questionnaire below in advance of your well-baby and child’s check-up at the Health Center (at the age of 4 months, 10 months, 18 months, and 3 years). Please describe your baby’s condition and your concerns.

For 5-year-olds, the Parent Checklist for 5-Year-Olds （5歳児健診チェックリスト）is sent at least one month before your child’s fifth birthday. Please complete the form and use it to consider whether your child needs a consultation at the health center. The five-year-old health checkup is optional, so if you would like a consultation, please call your ward’s health center in advance to make an appointment.
