13 Dec Do you need a vaccination record for your child?

If you are moving to another country, you may need to show vaccination (also called “immunization”) records for your child. You can make a request for the records in English at the health center at your local ward office in Sapporo. Fill out the record  application form below, and bring it to your health center. Be sure to bring your child’s maternal and child health handbook with you. The health center will check the handbook to confirm the vaccinations your child has received.

Cost: 400 yen (plus mailing fee￥84 if you wish to receive the record by surface mail)

Time required for issue: About 1 week
