25 Aug Free coupon(s) from Sapporo for cervical and breast cancer screening

If you are eligible, you might have received this notification from Sapporo  around late June, with free coupon(s) and a health check-up booklet for cancer screening test(s) enclosed.

If you receive two copies of this notice, check the expiration date on the coupon enclosed with each notice.
Please use the coupon with the correct expiration date. The invalid coupon has an expiration date of March 31, 2023, “令和5年3月31日まで” .
Valid coupons have an expiration date of March 31, 2024 “令和6年3月31日まで”.
 Discard invalid coupons.

All the free cancer screening tests must be completed by March 31, 2024.
You can only use the coupon(s) at designated medical institutions.
Please make appointments early since they are likely to be fully booked right before the expiration date, especially between February and March.

There are 2 types of screening tests.

For pap smear screening (including taking medical history, check-up, and pelvic exam.)
A free coupon will be sent to you around late June, 2023 if you are:
a registered resident of Sapporo as of April 20, 2023 and
a female, 20 years old as of April 1, 2023.
For breast cancer screening with mammography
A free coupon will be sent to you around late June, 2023 if you are:
a registered resident of Sapporo as of April 16, 2023 and
a female, 40 years old as of April 1, 2023.
