To obtain subsidies for children up to the age of 12 years, apply for infant and child medicare card (a pink card) at the same time as Registration of Birth.

Child Medical Expenses Subsidy 【子ども医療費助成】

Sapporo city offers medical subsidies for children up to the age of 12 years. However you have to pay for your first visit to the doctor. For example: pre-school children ¥580 for a hospital, visit and ¥510 for the dentist.
(For both outpatient visits and hospitalization)
You have to pay for some items such as containers for ointment and meals if your child is hospitalized.
This card expires. For children who do not enter elementary school yet, Sapporo city confirms parents’ income every July. If your child is eligible, you will receive a new pink card by the end of July. If not, you will receive a letter.

Child Allowance, Kodomo-teate 【子ども手当て】 Jidou-teate 【児童手当て】

This allowance is available for those who are raising a child of junior high school age or younger. You will get 15,000 yen per month if your child is under 3 years of age. Please apply at the ward office (kuyakusho) within 15 days of your baby being born.
Since the system has been changed in October 2011, you need to make another application.
You will receive a document by the end of October. Fill it out and submit it to the ward office (kuyakusho) soon.

In April 2012, Child Allowance “kodomo teate” has changed to “Children Allowance, Jido-teate.”.
Payment periods and qualifying ages are the same as the previous system, however there is a regulation on eligibility.

The new system requires a confirmation of household statuses and income. Please fill out and return the “current status information” paperwork (genjo todoke) that will be sent around the middle of June 2012.
If you fail to return the information, you will not be able to recieve Children Allowance after October.
